Summary

# Post-operative Ileus in Horses: Current Clinical Practice Amongst European Specialists Post-operative ileus (POI) remains a significant complication following equine abdominal surgery, yet consensus on its clinical definition and evidence-based management strategies has been lacking. Lefebvre and colleagues surveyed diplomates of the European Colleges of Equine Internal Medicine and Veterinary Surgeons to establish current practice patterns, discovering substantial variation in how specialists both characterise and treat this condition across European centres. Whilst pharmacological interventions were widely employed, the survey revealed that clinicians also rely on diverse supplementary strategies—including nasogastric decompression protocols, feeding regimens, and pain management approaches—that lack standardisation and published evidence. The findings highlight a genuine clinical need for prospective research evaluating which combinations of interventions most effectively resolve ileus and reduce complications such as reflux and re-colic, as current practice appears largely experience-driven rather than protocol-based. For practitioners involved in post-operative care, this work underscores the importance of adopting evidence-based guidelines once available, whilst recognising that optimal POI management likely requires integrated pharmacological and supportive strategies tailored to individual patients rather than a single therapeutic approach.

Practical Takeaways

  • POI management remains variable among specialists; consult current evidence and experienced colleagues when developing your POI prevention and treatment protocols
  • Pharmacological strategies continue to evolve—stay updated on emerging treatments beyond traditional approaches
  • Consider implementing comprehensive management strategies beyond medications, including physical therapies and supportive care, though further research is needed to define best practices

Key Findings

  • Survey of European equine specialists documents current clinical definitions and diagnostic approaches to equine post operative ileus
  • Multiple pharmacological strategies are in use for POI treatment, reflecting ongoing evolution in management protocols
  • Limited standardization exists regarding supplementary preventative and management strategies beyond pharmacological interventions
  • Study identifies gaps in evidence-based understanding of optimal POI management approaches
