Correlation between serum total globulins and gamma globulins and their use to diagnose failure of passive transfer in foals.

# Editorial Summary Failure of passive transfer (FPT) in foals remains a critical concern for neonatal survival, yet current diagnostic approaches often lack the sensitivity needed for reliable screening or prove too cumbersome for routine clinical use. Fouché and colleagues examined whether a simple calculation—total serum globulins (total protein minus albumin)—could effectively predict gamma globulin levels in 56 hospitalised neonatal foals, comparing results against electrophoretic measurement of gamma globulins as the reference standard. The researchers found a strong correlation (0.84) between total globulins and electrophoretic gamma globulins, with receiver-operator characteristic analysis yielding area-under-curve values of 0.887–0.930 depending on the diagnostic threshold applied; using appropriately selected cut-off values, total globulin measurement achieved ≥90% sensitivity for detecting clinically significant FPT whilst maintaining negative predictive values exceeding 94–97%. For practitioners, this suggests that total globulin measurement offers a practical, readily available screening tool for identifying foals at risk of FPT, reducing reliance on more time-consuming techniques and enabling faster clinical decision-making in neonatal cases where prompt intervention is essential.
