BPV DNA was detected in 95% of sarcoids at the 4 mm margin, declining to 73% at 8 mm, 39% at 12 mm, and 33% at 16 mm—a striking finding indicating subclinical viral presence in macroscopically normal skin; moreover, horses whose surgical margins tested positive for BPV DNA showed significantly greater likelihood of local recurrence over the 6-month follow-up period. These findings suggest that the traditional surgical approach of removing sarcoids with standard margins may be inadequate, as viral DNA frequently extends beyond visible disease boundaries, and integration of margin testing into surgical planning could potentially improve outcomes and inform decisions about adjunctive therapies such as topical chemotherapy or immunomodulation to address residual viral burden.
